Where can I find an electronic copy of a thesis/dissertation?

I am looking to find a copy of this thesis: Development and evaluation of a user friendly SAS program for cosinor analysis with applications to chronobiology
Author: Shan Lu
Publisher: 1999.
Dissertation: Thesis (M.S.)--University of Texas Health Science Center at Houston, School of Public Health, 1999.

I am trying to analyze chronobiological data in SAS and would appreciate being able to read this paper. Thank you.

I apologize that it took so long to get back to you.  I had to get permission from the author to allow us to copy it.  We can send you a paper copy or an electronic copy (scanned PDF), your preference.  The charge is .08 per page plus $20 handling. The thesis is 71 pages long.

Please let me know if you would like for us to copy this for you.  If so, let me know your preference (print or electronic).  We will also need billing information.

Thank you.
